Adult Preventative Plan

$336/year or 12 consecutive monthly payments of $28

Services covered yearly:

Periodontal Plan

$480/year or 12 consecutive monthly payments of $40

Services covered yearly:

  • 3 periodontal cleanings
  • 2 routine doctor exams
  • 1 set of 4 X-rays

Child Smiles Plan (13 and younger)

$300/year or 12 consecutive monthly payments of $25

Services covered yearly:

  • 2 professional cleanings
  • 2 routine doctor exams
  • 1 set of 2 X-rays
  • 2 fluoride varnish treatments

Payment is due at the time of service to qualify for the savings detailed above.
